would it be possible to add dummy images to missing images inside a content field?

so we dont end up with a ton of missing images inside the body field (yup theres still people that adds images with tinyimce etc)

Hmm, I guess that would be possible, but it would be some kind of input filter or preprocessing on the field. I am not 100% up to speed with how that happens in D7, but lemme look into it
