code validation shouldn't be mandatory when users have to be approved by admin

As of now, hash code validation is mandatory, even if the new account request has to be approved by user administrators. It should be made optional.

ATM, the only option is to manually call _ldapprov_code_submit() from a module to automate the validation and directly put the request to status 1 in ldapprov table.
