If you exclude some name components in the field settings of a name field, the "override shared field settings" dialog in the form display of this field look a little messy. More precisely it only seems to happen if the "credentials" are excluded.

If no components are excluded it looks ok.

Man, these templates are a bit messy, maybe some bigger refactoring to tidy up are on the cards, but not for today ;)
