I tested module with cck imagefield and imagecache;
I upload original images 2000x2000px and maked this settings with imagecache:presets_thumb (100 x100) and presets_medium (600x600);

but also if in display filed I setting for tesaer and full only this value

jlightbox:presets_thumb
jlightbox:presets_thumb gallery
jlightbox:presets_medium
jlightbox:presets_medium gallery

jlightbox open only original size (2000x2000) each time.

I use
drupal 5.7
ImageCache 5.x-1.6
ImageField 5.x-2.1

only a clarification (I see post you marked how original issue) but isn't clear some things (..also I not know good english):

why your module insert in the display fileds this options:
jlightbox:presets_thumb
jlightbox:presets_thumb gallery
jlightbox:presets_medium
jlightbox:presets_medium gallery

if after, when I assign at teaser and/or full display they not are used in jlightbox (see that it open only the original size) ?

or is necessary to use the sime's code patch (but how)?

in the 'orginal' Lightbox 2 module when one navigate in the gallery the images are not the original but is used a fixed size and there is a botton for enlarge to the original size; so navigaion is based on a fixed medium size; original is option's button that is activate if in display field one use for full the parameter original size
